Independent Living Coach/Homemaker
The Independent Living Coach is semi-professional position assisting people with disabilities to achieve the level of independence they desire by supporting them in setting and achieving goals and coordinating services. This is done by providing individualized assistance and coaching with personal care, meal preparation, leisure activities, transportation to jobs and appointments and helping with other daily activities in order to assist individuals in gaining independence and achieving personal goals. This job classification exercise considerable independence and reports to and receives direction from the Program Coordinator or their program designee.
Independent Living Coach duties include but are not limited to:
• Assist in teaching and training while supporting the consumer in meeting all independent living goals in accordance to their Coordinated Service and Support Plan (CSSP)/Care Plan.
• Ability to maintain consumer confidentiality in accordance to the Health Insurance Portability and Accountability Act (HIPPA) guidelines.
• Assist consumers to increase their awareness of community resources.
• Respecting the rights, values, and beliefs of the consumer.
• Maintaining all scheduled appointments.
• Maintain ongoing contact with your supervisor to make sure the consumer and Access North’s needs are continually being met.
• Complete all necessary personnel and consumer paperwork.
• Maintain Compliance with all of Access North policies and procedures.
• Demonstrate competency in each of the 245D (home and community based) services Access North provides.
• Reliable transportation, valid driver license and automobile insurance.
• Ability to work independently.
• Able to work flexible hours as scheduled.
• Other duties as assigned.

(In all cases, reasonable accommodations will be considered.) Much of the work day is spent in the consumers home or community as determined by the goals in the consumers CSSP/Care Plan.
• This position may require exposure to outdoor elements including dust, fumes, noise and glare.
• This position does require the majority of the work day spent sitting, standing and walking.
• Weights of objects to be lifted, carried, pushed, or pulled rarely exceed 24 lbs. Simultaneous use of hands, wrists, and fingers may be required.
• Movements such as climbing, stomping, crouching, twisting, bending, reaching, handling, fingering and feeling are sometimes required.
• Audio, visual, and verbal functions are vital aspects to performing this position.
The Homemaker Direct Support Professional (DSP) helps provide a safe and clean home by performing various tasks and companionship activities. Duties include but are not limited to:
• Cleaning, sweeping, vacuuming, taking out the trash, dusting, changing linens, dishes, mopping floors in the kitchen, bathroom and bedrooms (if applicable).
• Prepare light meals and/or snacks.
• Assist in arranging transportation for medical appointments, grocery shopping, and other community activities.
• Remind and assist the consumer with self-administered medications following Access North’s Medication Assistance Policy and Procedure.
• Other duties as assigned according to the consumers Care Plan.
Must be 18 years of age or older; provide proof that you are eligible to work in the U.S.; must pass a criminal background check; must hold a valid Driver's License and current automobile insurance, must have access to reliable transportation and be able to transport themselves to areas not assessed by public transportation.
• Ability to interact effectively and harmoniously with others.

• Experience working in the Health Care field.
• Knowledge of Independent Living Philosophy.
• Ability to work with diverse populations of people with disabilities in a professional manner.
• Demonstrates the knowledge and ability to follow all policies and procedures of Access North.
• Ability to access and maintain online internet communication for Access North's educational program.
• Knowledge of computer applications and computer software programs.
• Ability to communicate effectively both orally and in writing.
• Ability to establish and maintain effective working relationships.
• Ability to complete all necessary paperwork in a timely manner.
• Evidence of completing a Home Health Aide or Certified Nursing Assistant (CNA) Program is very desirable but not required.
